APL-NDPP supports GPs to prevent type 2 diabetes in their at-risk patients.
Using our APL-NDPP tool, GP practice teams can quickly list patients registered with them who are eligible for behaviour change programmes through the NHS Diabetes Prevention Programme, and can prepare a prioritised list of people to invite.
Practices run a single search in their clinical system and import the results into the tool to:
APL-NDPP supports an individualised approach to patient referral. The ‘interpreter needed’ and 'language' fields allow for coordination of language-based referrals across an area. Our Diabetes Prevention Programme provider can arrange language-specific groups if they receive referrals within a similar timeframe, and this has the potential to help address inequity in access. This is particularly important for diet and lifestyle education programmes, where cultural sensitivity can help increase the likelihood of behaviour change.— Dr Shaine Mehta, GP and CEG clinical lead
